Catalog Request Form
	We have a minimum order of US100.00 and sell in wholesale quantity for resale purpose only. If you are retailer or corporation promotional firm and can purchase in large quantity you are welcome to request a catalog by sending us an email at Om Imports.

California Resale Certificate Form
	All customers from within California are required to fill out a California Resale Certificate or you will be charged 9.25% sales tax.
